Practice 3 are committed to providing high-quality, patient-centred care. An important part of this is continuity of care, which means seeing the same GP or clinician for ongoing health concerns whenever possible. This helps your doctor understand your medical history, monitor your health over time, and provide care that is more personalised and effective.

What Is Continuity of Care?

Continuity of care means building an ongoing relationship with a GP or healthcare professional who becomes familiar with your health needs and coordinates your care over time. While it may not always be possible to see the same clinician at every appointment, we aim to offer this whenever we can, especially for patients who will benefit most.

Why It Matters

Seeing the same GP can help to:

  • Improve management of long-term conditions
  • Reduce the need for emergency or hospital care
  • Build trust and improve communication between you and your clinician
  • Provide more personalised and consistent care

Who Benefits Most?

Continuity of care is helpful for everyone, but it can be particularly important for patients who:

  • Have long-term conditions such as diabetes, asthma, COPD, or heart disease
  • Have complex or multiple health needs
  • Are older or frail
  • Need support with mental health or ongoing care

How You Can Help

You can help us support continuity by:

  • Requesting your usual GP when booking an appointment
  • Letting our reception team know if your issue is ongoing
  • Following up with the same clinician when advised

We will always do our best to offer appointments with your usual GP where appropriate, although this may sometimes mean waiting a little longer.

Providing safe, effective, and personal care is our priority, and continuity of care plays an important role in helping us achieve this.
